I recently had the pleasure of building out some advanced contact form functionality for Westside Medical.
The implementation features a request appointment custom CSS button with a onclick javascript event. This jumps to another page that features another button that utilizes twitter bootstrap to collapse and expand the contact form, complete with media queries to resize the buttons and text for mobile devices.
The Problem
Westside Medical wanted contact form submissions for a specific doctor to email to that doctor, rather than a catch all email address.
The Solution
- Added in request appointment menu option
- Directed users to a list of doctors or the general contact page.
- Select your desired doctor
- Each doctor page features it’s own contact form.
